What Is Pelvic Congestion Syndrome?

Like chronic venous insufficiency in the lower legs, pelvic congestion syndrome is characterized by an inability of the veins to return blood to the heart efficiently. In both cases, blood pools, causing swelling, pain, skin disturbances, and a range of other potential issues.

However, PCS can be significantly more painful than vascular insufficiency of the lower legs, and because this issue involves varicose veins deeper within the pelvis (near the fallopian tubes, ovaries, vulva, and other sensitive tissues and structures), it is often more difficult to identify.

What Causes Pelvic Congestion Syndrome?

Several factors can cause or exacerbate PCS, which most commonly affects women between the ages of 20 and 45. Some of these factors work together to increase your risk of developing this condition.

For example, if any of the following apply to you, you may be at a significantly higher risk of developing PCS:

  • Multiple pregnancies: The expanding uterus places pressure on pelvic veins.
  • Polycystic ovary syndrome: Increased estrogen weakens and dilates pelvic veins.
  • Heredity: Women with a family history of varicose veins are more likely to develop PCS.
  • Fullness in the leg veins: Dilated, dysfunctional veins encourage blood pooling.

What Are the Symptoms of Pelvic Congestion Syndrome?

Pelvic pain is the most common symptom of PCS and can sometimes be severe. Because PCS occurs so close to the reproductive system, several other symptoms can result as the enlarged veins impinge on and irritate nearby anatomical structures.

These additional symptoms include, but are not limited to, the following:

  • Swelling of the vulva
  • Visible varicose veins on or around the vulva or buttocks
  • Abnormal, painful menstruation
  • Pain during intercourse
  • Backaches
  • Vaginal discharge

Chronic PCS often leads to mental and emotional disturbances as well, including depression and lethargy. Many women with undiagnosed PCS report feelings of isolation, especially when being misdiagnosed or dismissed by healthcare providers without vascular health specializations.

How Is Pelvic Congestion Syndrome Treated?

Our ultrasound technology at the Vein Care Center allows the team to diagnose peripheral vascular disease, but when it comes to venous disease in the pelvis and abdomen, a CT scan is recommended for proper diagnosis of PCS. The team of excellent providers perform a physical assessment along with listening to your complaints to decide if a CT scan is warranted.

Once you are officially diagnosed with PCS, treatment options range from compression therapy, pelvic floor therapy, or a referral for an embolization of the pelvic veins  Many women find that addressing the root cause through venous treatments is the most impactful, both in alleviating PCS-related pain and improving quality of life after treatment.
