This condition develops when blood clots cause lasting damage to the valves and walls of deep veins, resulting in impaired blood flow and chronic symptoms in the affected limb.

The syndrome occurs because DVT causes inflammation and damage to the delicate valve structures within deep veins. These valves normally prevent blood from flowing backward, but once damaged, they become incompetent and allow blood to pool in the lower extremities. The resulting venous hypertension leads to increased tissue permeability, edema formation, and reduced arterial blood supply to the affected limb. Post-thrombotic syndrome affects 20% to 50% of patients within two years of their DVT diagnosis, with symptoms typically developing within the first six months but potentially appearing up to two years later.

How Is Post-Thrombotic Syndrome Diagnosed?

What Are the Symptoms of Post-Thrombotic Syndrome?

The most common symptoms include chronic pain described as aching, cramping, or throbbing that worsens with prolonged standing or walking. Patients often experience a feeling of heaviness or tiredness in the affected limb, along with persistent swelling that may fluctuate throughout the day but typically worsens toward evening.

Skin changes are frequent and may include discoloration, pigmentation changes, or a reddish-brown appearance due to the breakdown of red blood cells leaking from damaged veins. Many patients report itching, burning or abnormal sensations like pins and needles in the affected limb. In severe cases, venous ulcers may develop, typically appearing as open sores on the lower leg or ankle that heal slowly and may recur if not properly managed.
